I typically create my own lesson plans and try to cater them to the student. Most recently I've been working with trying to find a balance of teaching music theory and practical piano. When teaching theory, I often refer to Kotska's and Payne's "Tonal Harmony" for the order of the curriculum, and when it comes to exercises I will either write them out on the spot to demonstrate something to the student (I also feel it is important to SHOW the student how to MAKE music, ie. proper note notation, and symbol shape), or prepare them in advance if the content is more complex. I like to lay a foundation for what musical notes are and how they relate to the instrument. If we're on guitar I will draw a piano keyboard and give the student the guitar string names and make them figure out the rest of the notes. Then I teach them what chords are and, now that they know their way around a fretboard, they can build the chords themselves. While most students aren't total beginners and won't need all of this groundwork to be laid by me, I've found that almost everyone has gaps in knowledge, more often than not when it comes to actual note names and the music theory behind the instrument. I generate an individualized course of study for each student. Beginning students typically begin with the Faber method, to make sure they develop a strong foundation for future study. As they progress, I begin to introduce age-appropriate repertoire. My more advanced students focus on musicianship technique as well as repertory.
