I usually start with a beginners book for children or adults.  We go through the lessons at their own speed.  As for practicing the scales, I do not introduce them until the students is thoroughly familiar with the keys (the alphabet of the keys.)  For adults I introduce the scales and arpeggios much earlier. For beginning adults I try to determine exactly what they are wanting to achieve by starting an instrument at this point in life.
