Setting realistic goals is definitely a good place to start. Also, I encourage transcribing music in small sections, and to always start slow and take deep breaths, as this will reduce the student's normal anxiety and has done wonders in my own playing as well. Using a rhythm reference like a metronome or a drum loop while practicing is also a must, at it helps develop the student's groove. For beginning students who are children, I usually start with technique exercises that will help develop the foundation of their playing. I do not introduce music notation usually until the fifth or sixth lesson. I believe that kids learn faster when they beginning learning by call and response or like many call imitation. Especially with adult students I think it is essential to cater to the students' goal. If this involves the need to learn certain techniques, requires knowledge of music theory or the ability to read music, I will advise the student so.
